For our team to ready your insurance benefits prior to your first visit, many insurance carriers utilize patient social security number and date of birth as identifiers for verification of benefits.  Other insurance carriers will assign members separate identification member numbers.  Your patient information will be handled with the highest security and privacy; however, if you are uncomfortable with releasing social security information on our secure forms, please contact your PPO insurance carrier prior to your visit and obtain your unique member identification number for us to verify benefits and eligibility.  If this information is missing on your secure form, we will not be able to verify your insurance benefits.
